Combined Approach in Breast Augmentation: How Are Implants and Fat Injection Used Together?

Breast augmentation surgery is a frequently preferred procedure for increasing volume and improving shape. While silicone implants are used in the classic approach, “combined” techniques supported with fat injection (lipofilling) have become more prominent in recent years. In this approach, the aim is not only to add volume, but also to soften transitions, make the contour more balanced, and make fine adjustments in certain areas.

Using implants and fat injection together is not necessary for every patient. However, in certain anatomical situations, it may help complete details that are difficult to achieve with implants alone. Therefore, the combined approach is not a standard method; it is a need-based planning option.

What Is Combined Breast Augmentation?

Combined breast augmentation is based on the principle of supporting the main volume provided by silicone implants with the patient’s own fat tissue. This method means using two different techniques together: implant placement and fat transfer.

The implant is the main element that determines breast volume. Fat injection is used more at the superficial level, for contour adjustment and softening transitions. Therefore, fat injection does not replace the implant; it plays a complementary role.

This approach may be preferred especially in the following situations:

● Camouflaging implant edges in patients with thin skin structure
● Creating a more natural fullness in the upper pole, the upper part of the breast
● Correcting asymmetries
● Making the décolleté area have softer transitions

This technique makes it possible to work on both volume and surface contour.

What Does a Silicone Implant Provide on Its Own?

Silicone implants are the method that provides the most predictable volume increase in breast augmentation surgeries. The size, shape, and projection of the implant can be determined in advance, and the result can be planned accordingly.

With implants:

● Noticeable volume increase is achieved
● Breast shape is reshaped
● Symmetry can be adjusted

However, in some cases, the borders of the implant may be noticeable. Especially in people with thin skin structure or in cases where breast tissue is limited, the edges of the implant may be felt or noticed from the outside. At this point, fat injection comes into play to make these transitions softer.

How Is Fat Injection Used in Breast Augmentation?

Fat injection is the process of transferring fat tissue taken from different areas of the body, such as the abdomen, waist, or hips, through liposuction to the breast area after special processing.

In this method:

● Fat tissue is purified
● It is injected in thin layers
● It is distributed to areas with suitable blood supply

Fat injection can be used alone for breast augmentation; however, the volume that can be achieved is limited. Therefore, in cases where more noticeable enlargement is desired, using it together with an implant offers a more balanced approach.

Some of the fat is absorbed by the body over time, while the remaining portion becomes permanent. For this reason, this process is taken into consideration when planning the injection.

What Advantages Does the Combined Approach Provide?

Using implants and fat injection together allows some details that cannot be achieved with implants alone to be adjusted.

The main contributions of this approach include:

● Making implant edges less noticeable
● Softer and more natural transitions
● More balanced fullness in the décolleté area
● Correction of small asymmetries

Is It Suitable for Every Patient?

Combined breast augmentation is not necessary for every patient. For this approach to be applied, some basic conditions must be met.

Usually, fat is first removed with liposuction, then the implant is placed, and fat injection is performed in the final stage. This sequence allows the tissues to be shaped in a more controlled way. There is no standard plan for every patient. Which areas will receive how much fat is determined entirely according to individual anatomy.

Is the Recovery Process Different?

In the combined approach, the recovery process includes both breast surgery and the areas where liposuction has been performed. Therefore, compared to implant surgery alone, there is a slightly broader recovery area.

During the recovery process:

● There may be sensitivity and a feeling of tightness in the breast area
● Bruising and edema may be seen in the areas where fat has been removed
● Compression garments or support products may be used

Pain is generally manageable and decreases over time. The time required to return to daily activities may vary depending on the scope of the procedures performed. Since the permanence of fat injection becomes clearer over time, the final result appears within a few months.

The Balance Between Naturalness and Volume

One of the most important goals in the combined breast augmentation approach is to achieve a balanced result between volume and naturalness. The implant alone provides noticeable volume; fat injection contributes to this volume appearing more natural.

However, this balance is different for every patient. Some patients want more noticeable volume, while others may prefer an appearance with softer transitions.

For this reason, during the planning process:

● Volume expectation
● Skin quality
● Existing breast tissue

should be evaluated together. When used correctly, the combined approach helps establish this balance.

In Which Cases Is an Implant Alone Sufficient?

The combined approach is not necessary in every breast augmentation surgery. Especially in patients with sufficient breast tissue and suitable skin structure, an implant alone may provide a satisfactory result.

Situations where an implant alone is sufficient include:

● Moderate volume need
● Good skin quality
● Minimal contour irregularity

In such cases, additional intervention may not be necessary. The combined approach comes to the forefront mostly in cases requiring fine adjustment.

Which Approach Is More Suitable?

There is no single correct method in breast augmentation surgery. Implant, fat injection, or the combined use of these two methods is determined according to the patient’s needs.
