Which Aesthetic Procedures Can Be Combined to Create an Hourglass Body Shape?
There is no single aesthetic procedure that creates an hourglass body shape. This appearance is generally planned by making the waist more defined, balancing the transition between the hips and buttocks, correcting excess skin or fat in the abdomen, and, when necessary, adjusting breast proportions to better harmonise with the rest of the body. For this reason, liposuction, BBL, tummy tuck, breast surgery, and skin-tightening methods in suitable patients may be considered in different combinations.
However, it is not anatomically possible for everyone to achieve the same hourglass proportions. The width of the rib cage, pelvic structure, natural position of the buttocks, fat distribution, waist length, and skin quality all determine the basic limits of the body silhouette. The aim of surgery is not to create a standard body shape, but to improve the balance between the waist, hips, and torso within the individual’s existing anatomy.
Which Anatomical Features Determine an Hourglass Body Shape?
An hourglass appearance depends not only on having a narrow waist, but also on having a defined transition between the waist and hips. A wider pelvis or naturally prominent buttocks may support this proportion in some people, while the potential to create the same appearance may be different in those with a narrower pelvis. Similarly, a wider rib cage or shorter waist may affect how narrow the waist can appear.
The distribution of fat throughout the body is also important. If there is significant fat around the waist and back while buttock volume is already adequate, circumferential liposuction alone may create a meaningful change in proportions. If buttock volume is limited or hip dips are prominent, narrowing the waist alone may not produce the desired silhouette.
How Is Liposuction Used to Create an Hourglass Appearance?
Liposuction is one of the procedures most commonly considered when changing the waist-to-hip ratio. The aim is not simply to remove fat from the abdomen, but to shape the circumferential contour of the torso by assessing the waist, flanks, back, and, when necessary, the lower back together. This approach is sometimes referred to as 360-degree liposuction.
Liposuction does not change bone structure and does not surgically remove skin. Therefore, fat removal alone may not be sufficient in a patient with significant excess skin or abdominal wall laxity. In addition, if skin elasticity is poor, reducing too much volume may make existing laxity more noticeable.
How Can BBL Change the Waist-to-Hip Ratio?
BBL involves preparing suitable fat obtained through liposuction and transferring it to the buttocks. In an hourglass body plan, it may be used not only to enlarge the buttocks but also to balance the transition between the waist and hips by adjusting volume distribution in the upper, lateral, or central areas of the buttocks.
Particularly in some patients with limited buttock volume or prominent hip dips, narrowing the waist with liposuction and supporting the buttock contour with fat transfer may be considered together. However, the amount of fat that can be transferred depends on the patient’s available fat reserves and tissue capacity. Keeping fat injection within the safe subcutaneous plane is one of the fundamental surgical safety principles of BBL.
When Is a Tummy Tuck Added to an Hourglass Body Plan?
A tummy tuck is not a procedure performed to reduce fat around the waist. Its primary purpose is to remove significant excess abdominal skin and, when necessary, repair separated abdominal muscles after pregnancy or weight changes. For this reason, it may form part of an hourglass contouring plan in patients with loose skin and abdominal wall concerns.
A tummy tuck may often be planned together with liposuction of the waist and flanks. In this way, excess skin on the front of the abdomen can be corrected while the side contours of the torso are also shaped. However, in a patient with good skin quality, no muscle separation, and localised fat as the main concern, liposuction alone may be sufficient instead of a tummy tuck.
In Which Patients Can Quantum RF Be Added to the Combination?
How well the skin adapts to the reduced volume after liposuction depends on its elasticity. In suitable patients with mild to moderate laxity, internal radiofrequency-based methods such as Quantum RF may be considered together with fat reduction to support tightening of the skin and subcutaneous tissues.
However, this method does not replace the removal of significant excess skin. If there is severe abdominal laxity forming folds or excess skin after major weight loss, surgical skin removal may be more appropriate. Therefore, combining liposuction with Quantum RF is particularly relevant in patients with limited skin excess.
Can Breast Surgery Affect Overall Body Proportions?
An hourglass appearance is not determined only by the waist and buttocks. The proportion between the upper and lower body also affects the overall silhouette. If there is significant breast sagging, volume loss, or breast volume that is disproportionately large for the body, procedures such as breast lift, augmentation, or reduction may be included in the body contouring plan.
The aim is not necessarily to enlarge the breasts. In some patients, the existing volume is sufficient and only a lift is needed. In others, an implant or fat transfer may be considered if volume is lacking. Breast surgery should not be planned mechanically according to hip or waist measurements, but according to overall body proportions and the patient’s expectations.
How Is the Appropriate Combination Determined?
The right combination is determined by first identifying which anatomical features are limiting the desired hourglass appearance. Liposuction may be considered for excess fat around the waist, BBL when buttock volume or transitions need support, tummy tuck for significant excess abdominal skin, and methods such as Quantum RF in suitable patients with limited skin laxity.
In some people, liposuction of the waist and back alone may be sufficient, while another patient may require a combination of tummy tuck and liposuction or liposuction and BBL. The aim of surgical planning is not to copy a particular image from the internet, but to create a more balanced waist-to-hip ratio within the limits of the patient’s rib cage, pelvis, skin quality, and existing fat distribution.
