Proportion, Balance and Real Anatomy: What Can Artificial Intelligence Not See in Body Contouring?

Artificial intelligence tools can analyse an uploaded body photograph within seconds and provide comments on areas such as the waist, abdomen, breasts or buttocks. Some tools can also generate new images showing how a person might appear after an aesthetic procedure. However, what artificial intelligence evaluates is not the actual body, but a two-dimensional image of the body captured at a particular moment and under specific conditions.

Planning body contouring procedures is not based solely on the contours visible in a photograph. Skin quality, the location of fat tissue, muscle structure, skeletal characteristics, posture, movement and the person’s medical history must all be evaluated together. Therefore, although artificial intelligence may analyse an image, it cannot determine which aesthetic procedure a person needs.

Can a Photograph Show Real Anatomy?

Photographs are affected by lighting, camera angle, the lens used, shooting distance and the person’s posture. The same person may appear slimmer, broader, more symmetrical or disproportionate in different photographs. Even tightening the abdomen, rotating the waist or positioning the buttocks further back can alter the appearance of the body contours.

Artificial intelligence can examine what is visible in a photograph, but it cannot always determine how accurately the image reflects the person’s actual anatomy. A protrusion in the abdominal area may be associated with excess skin, subcutaneous fat, intra-abdominal fat, muscle separation or postural characteristics. Similar appearances may be explained by entirely different causes during a physical examination.

Can Artificial Intelligence Assess Tissue Quality?

The external appearance of the body reflects only part of the structures beneath the skin. The relationship between the skin, fat tissue, muscles, connective tissues and skeletal structure plays an important role in determining which procedure may be appropriate.

Differences in Skin and Fat Tissue

The thickness, elasticity and degree of laxity of the skin, as well as its relationship with the underlying tissues, cannot be fully assessed from a photograph alone. During a physical examination, a plastic surgeon not only observes the skin but also evaluates it by touching, pinching and moving it in different directions. Artificial intelligence cannot determine how the skin feels or how effectively it may retract after a procedure.

The same limitation applies to fat tissue. An increase in abdominal volume visible in a photograph does not mean that the entire volume can be reduced through liposuction. It is necessary to determine whether the fat is located beneath the skin or within the abdominal cavity. Artificial intelligence cannot reliably make this distinction from a photograph alone.

Muscle Structure and the Abdominal Wall

Separation of the abdominal muscles may occur, particularly after pregnancy or significant weight changes. In some patients, the possibility of a hernia may also need to be evaluated. Artificial intelligence may generate an interpretation based on external appearance, but it cannot examine the muscle structure or establish a diagnosis.

When deciding between a tummy tuck, liposuction or a combined procedure, the external appearance is not the only consideration. The condition of the abdominal wall, the amount of excess skin and the distribution of fat tissue must be assessed together.

A Static Image Cannot Show How the Body Moves

The body is not merely a static image. The position of the shoulders, the curvature of the spine, the alignment of the pelvis and the way a person distributes weight across the legs can all influence body contours. An asymmetry that is not noticeable while standing may become more apparent while walking or bending forwards.

Some features that appear to be localised fat deposits or contour irregularities in a photograph may be related to postural habits. Artificial intelligence analysing a single image cannot assess how the body changes during movement or how different anatomical regions influence one another.

Are Popular Body Measurements Suitable for Everyone?

A pronounced waist-to-hip transition, greater buttock volume or specific breast dimensions are commonly seen on social media. Artificial intelligence may also generate suggestions that treat these recurring features in its training images as ideal. However, a popular appearance may not be suitable for every body type.

Shoulder width, rib cage structure, pelvic anatomy, leg length and overall skeletal proportions vary from person to person. For this reason, the same breast or buttock volume may look entirely different on two individuals. The aim of body contouring is not to apply fashionable measurements to every patient, but to create a balanced plan within the individual’s own anatomy.

Are Artificial Intelligence Simulations Realistic?

Images generated by artificial intelligence may help visualise a possible change, but they do not guarantee the appearance that will be achieved after surgery. In a digital image, the waist can easily be narrowed, volume can be added to the buttocks or excess skin can be removed. In actual surgery, however, tissue characteristics, blood supply, wound healing and safe surgical limits must all be taken into account.

The fact that an image appears realistic does not mean that the changes it presents are medically achievable. Artificial intelligence simulations should therefore not be regarded as a definitive surgical plan or as a target that must be achieved.

How Should Artificial Intelligence Be Used?

Artificial intelligence may be used to obtain general information about body contouring, understand medical terminology and prepare questions before a consultation with a surgeon. However, it cannot replace a physical examination or the clinical assessment of a plastic surgeon.

A person’s suitability for a particular procedure is determined by evaluating photographs together with tissue characteristics, movement, medical history, expectations and surgical safety. Artificial intelligence may see the image, but it cannot fully understand the living, moving and individual anatomy behind it.
