How Has Artificial Intelligence Changed Expectations About Aesthetic Surgery?

Artificial intelligence has made expectations about aesthetic surgery faster, more visual, and more personalised. A person can now alter their own photograph within seconds: making the nose smaller, narrowing the waist, increasing buttock volume, or transforming facial features into an entirely different form. These images may make it easier for individuals to explain the changes they want. However, they may also cause surgically impossible images to be perceived as realistic goals.

Artificial intelligence can recreate a photograph, but it cannot reliably assess a person’s skin quality, bone structure, muscles, fat distribution, wound-healing capacity, and medical history based on an image alone. Therefore, an AI-generated image is neither a guarantee of how someone will look after surgery nor a personalised surgical plan. It may shape expectations, but whether those expectations are anatomically realistic should be determined by a plastic surgeon.

Why Have Aesthetic Expectations Become More Visual?

In the past, patients might bring a photograph of a celebrity or another patient they admired to a consultation. Today, artificial intelligence tools can alter the individual’s own photograph directly, creating a much more convincing image. Because the person sees their own face or body on the screen, they may perceive the image not as a distant example, but as an achievable future appearance.

This can facilitate communication. Instead of saying, “I want a more natural-looking nose,” the patient may be able to demonstrate the type of change they are considering in the bridge, tip, or profile. However, the more realistic the image appears, the more likely the individual may be to become attached to it. The American Society of Plastic Surgeons has noted that even when patients are clearly informed that a computer-generated image may not be achievable, they may still adopt it as the expected outcome.

Why Are AI-Generated Images Not Surgical Simulations?

General-purpose image generation tools are not designed to provide surgical accuracy. These systems generally aim to create visually polished, symmetrical, and appealing images. In doing so, they may fail to account for excess skin, incision locations, scars, swelling, tissue thickness, and anatomical limitations.

A study published in 2025 reported that a substantial proportion of AI-generated aesthetic surgery images contained anatomically unrealistic changes and inaccurate postoperative predictions. The researchers stated that such images should not be used for surgical decision-making and that standardised photographs of real patients should be preferred during consultations.

For example, artificial intelligence may flatten the abdomen while making the skin perfectly smooth, failing to show anatomical changes to the belly button and completely ignoring a tummy tuck scar. In a breast image, it may create a volume that the existing tissues could not realistically support. On the face, it may remove pores, natural asymmetries, and age-related characteristics. The resulting image may be aesthetically impressive but surgically unachievable.

Have Filters and Artificial Intelligence Raised the Threshold for “Perfection”?

Artificial intelligence and advanced filters may change not only expectations about how surgery should look, but also perceptions of normal anatomy. When poreless skin, perfectly symmetrical faces, uninterrupted body contours, or scar-free surgical results are repeatedly displayed on screens, natural features may begin to be perceived as problems.

An experimental study published in 2025 reported that exposure to AI-enhanced photographs could increase expectations regarding plastic surgery outcomes and potentially contribute to lower postoperative satisfaction. Reviews of AI beauty filters have also emphasised that the psychological effects of these tools on body image and expectations regarding aesthetic procedures should be considered carefully.

The issue is not the desire for aesthetic change. The issue is comparing real human tissue with a smooth and static image generated by a computer. Surgical recovery involves stages such as swelling, temporary asymmetry, scar maturation, and tissue settling.

Is Artificial Intelligence Moving Everyone Towards the Same Standard of Beauty?

Artificial intelligence models may reproduce facial and body features that appear frequently in the visual data on which they were trained. This may lead characteristics such as a narrow nose, defined jawline, smooth skin, slim waist, or a particular waist-to-hip ratio to be repeatedly presented as the “ideal” appearance. Generated images may not represent different ages, skin tones, ethnic characteristics, and natural anatomical diversity equally.

Current research suggests that generative artificial intelligence systems used in aesthetic medicine may contain biases in terms of cultural sensitivity and representation. Therefore, an appearance frequently generated by artificial intelligence should not be assumed to be universally natural, healthy, or suitable for a particular individual.

The aim of plastic surgery is not to make patients conform to a common template created by technology, but to plan a balanced and safe change within the individual’s own anatomy.

Can Artificial Intelligence Be Useful During a Consultation?

When used within appropriate limits, artificial intelligence may support patient education, visual communication, and certain planning processes. Systematic reviews of artificial intelligence in plastic surgery indicate that applications are developing in areas such as image analysis, outcome prediction, measurement, and clinical workflows. However, many of these tools still require careful use in relation to clinical validation, data quality, privacy, and bias.

A patient may bring an AI-generated image to the consultation. This image may serve as a communication tool that helps the surgeon understand the desired change. Rather than attempting to copy the image, the surgeon should explain which features are being requested, whether they are anatomically achievable, and what risks may be involved.

If simulation is used, it should also be explained clearly that it is not a guaranteed postoperative photograph. Technology may support the consultation, but it cannot replace physical assessment and surgical experience.

How Should Realistic Expectations Be Developed?

Realistic expectations do not mean that a person should not have aesthetic goals. They mean aligning those expectations with real anatomy, surgical limitations, and the recovery process. For this purpose, standardised before-and-after photographs of real patients with similar anatomical characteristics may be more meaningful than AI-generated images. It should also be remembered that differences in camera angle, posture, lighting, and digital editing may significantly alter the appearance in photographs.

The consultation should not focus only on the question, “Can I look like this image?” Questions such as “What changes are possible with my anatomy?”, “Which features will not change?”, “Where will the scars be?”, and “When can the final appearance be assessed?” should also be discussed.

Artificial Intelligence Can Illustrate Expectations, but It Cannot Determine Surgical Reality

Artificial intelligence has made expectations about aesthetic surgery more visible and detailed. It has made it easier for people to communicate what they want, but it has also normalised flawless, scar-free images that exist independently of anatomical limitations.

The healthiest approach is to view an AI-generated image not as a definitive target that must be achieved, but as a reference that starts the conversation. Aesthetic surgery planning should be based not on the image on a screen, but on physical examination, the individual’s tissue characteristics, general health, and safe surgical limits. Artificial intelligence can generate an image of a possibility; it cannot determine which change is truly achievable or appropriate for the individual.
